Age Info Survey
In support of the 2009 Needs Assessment Project, The Massachusetts Office of Elder Affairs will be launching a general public questionnaire on Monday, August 18, 2008, via 1-800-AGEINFO. Persons interested in responding to the survey can access the questionnaire from http://www.800ageinfo.com/ , by clicking on Survey of Elder Service Needs-Your Voice Counts (in blue) located on the home page.
The questionnaire will be posted on 1-800-AGEINFO from August 18, 2008 through November 16, 2008, and I encourage you to market the questionnaire to your local Planning and Service Area via newsletters, menus, local needs assessment events, meetings, etc. Please note that upon completion of the questionnaire, the respondent can observe the current results of the survey.
