My Life My Health - Chronic Disease Self Management (CDSM)

For those with chronic physical and/or emotional challenges that impact quality of life, including diabetes, arthritis, weight issues, high blood pressure, heart disease, COPD and other illnesses. Learn how to manage pain and fatigue, understand nutrition and exercise options, improve communication with medical providers and family, use effective relaxation and breathing techniques, manage medications and make sound treatment decisions.

Workshops are highly interactive, provide mutual support and build participant’s confidence to manage their health and remain active. You will set goals and create and accomplish weekly “action plans” in a very supportive, fun atmosphere. Workshops meet weekly for six weeks.

Upcoming community workshops:

  • YMCA in Greenfield: Tuesdays, June 26 through July 31 from 12:30-3:00 pm at the YMCA. Lesley Kayan, Healthy Aging program director, and Ann Marie Meltzer will co-lead. Caregivers or family members are welcome, too!
